Date of valuation. 6. Identification of appraiser (name, firm, and address). 7. Project – Additions to Henry Hudson Trail B. Letter of Transmittal 1. Market value. 2. Date of valuation. 3. Appraiser’s signature, S.C.G.R.E.A. number and date signed. 4. Information pertinent to the report. C. Table of Contents 1. Reference required sections to specifically identified pages each consecutively numbered.
