Prospective bidders are required to provide pricing for the following requirements: 1. A single drug deactivation disposal pouch per mailing. The pouch must include clear instructions for use. 2. A customized resource card. The awarded vendor shall provide a proof and color match print for review prior to production. The resource card shall be tent-folded around the disposal pouch. 3. Resource cards shall include Opioid misuse prevention and education messaging including harm reduction resources, safe medication storage tips, community resources, linkage to Monmouth Acts ResourceConnect, and the QR code for the Monmouth County Opioid Settlement Community Survey. 4. Bidder’s pricing shall include the costs of printing and mailing each unit in addition to the total cost for all senior households (65+) within the identified zip codes. 5. Awarded vendor is responsible for obtaining all residential addresses registered to those 65 and older, including P.O. Boxes, for zip codes 07735 and 07753 in Monmouth County. The County reserves the right to add additional zip codes, and the Purchase Order will be amended to accommodate any additional quantities.
