1. The Consultant shall explain all aspects of each service/test above including what means and methods will be used throughout the evaluation process. 2. The Consultant shall schedule and complete the assessment and information gathering component of all evaluations within three (3) weeks of request by the County of Monmouth. The final analysis and results shall be provided to the using county agency as follows: a. Verbal results within twenty-four (24) business hours of assessment completion date. b. Written results or report within three (3) business days of assessment completion date, to include the New Jersey Psychological Certification Form (PTC-9), when applicable. 3. The Consultant shall perform the physical assessment and any other appropriate information gathering components of the evaluation process within Monmouth County, or in another mutually agreed upon location. If the Consultant has a facility within the Freehold area under a separate contract which permits the Consultant to utilize that space for evaluations conducted for other entities, then that would also be permissible so long as the County of Monmouth assumes no liability for the use of that space in any manner. The Consultant shall clearly indicate within their proposal the location they anticipate utilizing for all County evaluations. 4. The Consultant may propose the utilization of telehealth during any phase of the assessment and information gathering component of the evaluation process. If the Consultant proposes the use of such virtual processes, then the proposal shall detail when, where and how such technology will be utilized within the full evaluation process. 5. The Consultant should detail all aspects of each evaluation type, including but not limited to; history gathering, interviewing, clinical review, criminal/MVA abstracts, financial abstracts, testing instruments utilized and scoring. 6. The Consultant shall propose an all-inclusive unit cost for each evaluation type requested within this RFP. The price should include (as infrequent as they are) all costs associated with appeals made by the candidate/employee and defending their determination and processes. 7. The Consultant shall provide with the proposal a reference list of all current “law enforcement” concentrated contracts for psychological services. The list shall include agency name, address, telephone number, contact person, brief service description (e.g. pre-employment, fitness for duty) and start date of contract. 8. The Consultant shall provide with the proposal a reference list of all terminated or prior “law enforcement” concentrated contracts for psychological services going back ten (10) years. The list shall include agency name, address, last known telephone number and contact person, service type, start and end date, and reason the contract was terminated. 9. The Consultant shall utilize at minimum a N.J. licensed doctoral level psychologist with a major focus of work in the law enforcement field. It is preferred that the Consultant be N.J. Board certified in Police and Public Safety Psychology. A copy of the psychologist(s) resume(s) shall be provided with the proposal. 10. The Consultant should indicate any minor variance or exception to the specifications, terms and/or conditions; which shall be reviewed by Monmouth County and factored into the overall evaluation process. Substantial variances or exceptions may result in rejection of the proposal. The Consultant shall clearly indicate or articulate each and every variance, deviation or alternative, regardless of how slight. Monmouth County reserves the right to accept or reject any and all variances, deviations or alternatives. Unless the County of Monmouth specifically acknowledges as part of the contract award, any variances and alternatives shall be deemed rejected and the contract will be awarded based on the full compliance with the specifications as written by Monmouth County. 11. The Consultant should carry professional liability insurance with minimum $1,000,000 per incident and $2,000,000 aggregate. The policy must cover the Consultant, the firm and all its employees. A copy of all appropriate insurance certificates required here shall be provided before the contract start date. The policies shall run continuously throughout the contract term and written cancellation notice shall be provided to the county within thirty (30) days of the effective date. 12. The Consultant shall defend, indemnify and hold harmless the County of Monmouth, the Monmouth County Board of Commissioners, the Monmouth County Sheriff’s Office, the Monmouth County Prosecutor’s Office, the County’s Constitutional Officers (Sheriff, County Clerk & Surrogate), and all their respective servants, volunteers, and agents (collectively, for the purposes of this provision, the “County”) from and against all third-party claims asserted against the County arising from the Consultant’s negligence, willful misconduct, and/or its unconstitutional actions. The Consultant shall have no obligation to defend, indemnify, and hold the County harmless from any third-party claims raising out of the negligence, willful misconduct, and/or the unconstitutional actions of the County. 13. The Consultant should describe or include any other relevant or unique qualifications.
