1. Each neuropsychologist furnished by the provider must be capable of providing a neuropsychological evaluation and written reports on an as-needed basis. The neuropsychologist will be responsible to provide the following range of services: o Conduct neuropsychological evaluations for students identified by the District on an as-needed basis. o Write a neuropsychological report and provide feedback as required by the District. o Provide all required documentation to District personnel. 2. Provider shall, at no additional cost to the District, provide all necessary administrative paperwork, including billing information on District-required forms. The provider shall include the Districtassigned purchase order number on every invoice submitted for payment. Invoices for payment must be submitted within sixty (60) days of the date of service delivery. 3. The District may make such investigations as it deems necessary to determine the ability of the Proposer to perform the work. The Proposer shall furnish any information and data for this purpose as the District may request. The District reserves the right to interview proposers to determine the ability to satisfactorily perform the services. 4. The Provider and each neuropsychologist shall maintain adequate records of all Services, including neuropsychological evaluation reports. Such records shall be the property of the Board and shall remain confidential.
