Specifications include, but are not limited to: • Facilitate substance abuse evaluations for individuals housed in general population requesting substance abuse services through the program. • Facilitate individualized treatment planning addressing specific inmate clinical needs • Coordinate multi-departmental treatment plan review sessions. o Coordinate after care planning o Provide individual counseling and case management • Facilitate group therapy • Provide ongoing progress reports to legal system on inmates in treatment • Serve as inmate liaison to DYFS and Camden County Drug Court • Facilitate addiction education classes • Provide clinical coordination with Camden County Judiciary Teams • Facilitate discharge planning process • Provide ongoing treatment documentation • Coordinate referrals for Wrap Around Services for both in-house and community services • Coordinate mental health services with Center for Family Guidance (CFG) for individuals presenting with co-occurring disorders • Coordinate literacy services (Adult Basic Education, GED, ESL, Post Secondary and Vocational Training) • Coordinate HIV/AIDS, STD and TB education services • Facilitate linkages with Twelve Step Mentors/Sponsors
