▪ PS 23 & 29 is embarking on a full-service health clinic to provide physical and mental health services to the students in Pre-K – 8 th grades. The selected provider must provide a Treatment Coordinator that works 5 days a week with supervision. The provider must also provide a License Clinical Social Worker, 2 day a week to provide Behavioral Health services. The behavioral health/mental therapist will be providing individual and group therapy. The provider will be responsible for enrolling students into the health center. ▪ Dental visits for students must be provided every 6 months unless there is a dental emergency. Eye exams be provided yearly except if a child is experiencing problems with current prescription. These services will be provided monthly. Dental and eye professionals will see a minimum of 15 students the days they are providing services. ▪ The selected provider will provide monthly health and wellness workshops to parents/guardian focused on nutrition, wellness, etc. ▪ The selected provider will provide monthly reports to include enrollments, number of students/parents served, etc. ▪ The provider of service will provide a schedule will be approved by the Principal.
