The vendor will provide behavioral health consultative staff for Phase 2 of three (3) improvement initiatives: 4.1 CLINICAL CONSULTING & MEDICAL TRAINING MANDATE – PHASE 2 The vendor will provide behavioral health consultative staff for Phase 2 of three (3) improvement initiatives (Safewards; Centralized Medical Education, Training and Leadership; and Continued Extension Pending Placement (CEPP) Discharge Planning) and Project Management: 4.1.1 SAFEWARDS • Monitoring, implementation and assessment of successes and barriers to a sustainable model of care per Key Performance Indicator (KPI) throughout the State hospital system. • Recommendations for continuous quality improvement. 4.1.2 CENTRALIZED MEDICAL EDUCATION, TRAINING AND LEADERSHIP • Medical Bylaws modernization and related documents monitoring and support. • Provide a report on the recommendations for the Medical Service Organization including revised policies as agreed upon with Medical Staff and DOH leadership, Development of a performance tracking mechanism for the Medical Staff in compliance with Joint Commission and CMS standards/regulations. • Medical Education coaching and mentoring for court and testimony preparation. • Provide medical staff with training resources, tools and interactive coaching, including specialized forensic medical training. • Report out per KPI. 4.1.3 CEPP DISCHARGE PLANNING • Suggesting areas of improvement, implement training and monitoring initiatives pursuant to status patients and related discharge planning. • Evaluating interdisciplinary treatment plan reviews for discharge readiness. • Assessment of hospital admissions unit process and workflow redesign recommendations facilitating a KPI of 45-day discharge. • Recommendations for discharge preparation and KPI monitoring. 4.1.4 PROJECT MANAGEMENT • Coordinating with DOH staff on the above initiatives.
