The CONSULTING LABORATORY & TESTING SERVICES will consist of the following tasks: • Reviewing the orders for the laboratory tests from the patient's clinician(s). • Collecting specimens. • Labeling the collected specimens. • Recording the number and kind of specimens collected. • Recording the number of specimens to ensure that the test requisition forms are properly completed (test orders, billing information, patient and physician signatures) pursuant to orders from the patient's physician(s). • Causing adequate and sanitary supplies and equipment related to the Collector Services to be maintained at the facility. • Causing the adequate and sanitary disposal and storage of hazardous waste (derived from the specimen collection procedures) maintained at the Facility. • Taking steps necessary to ensure the integrity of specimens that are stored at the Facility. • Taking steps to ensure that the medical reports related to the specimens collected at the Facility are delivered to the physician(s) at the Facility in a timely manner. • Lab shall perform all Lab Services in compliance with any standard, ruling or regulation of New Jersey Clinical Laboratory Improvement Act, N.J.S.A. s 45:9-42.26 et seq., the federal Clinical Laboratory Improvement Act, 42 C.F.R. 493.1 et seq., or any other governmental agency responsible for administering, regulating or accrediting healthcare facilities or professionals. • Lab shall provide Client, with 2 Collectors for the provision of Collector Services, and pay the fees for said Collectors hours at the Facility. Lab will provide qualified alternative Collector, in the event that the scheduled Collector is not able to present for duties. • Lab shall provide to the Facility, at no extra cost, routine supplies necessary to submit samples for testing. • At a minimum the consulting laboratory shall meet the following requirements: • Clinical Medical Laboratory with all required Accreditations / Certifications • Accredited by the College of American Pathologists (CAP) o CLIA Certified • Certified Phlebotomists and/or Collector (Preferably Bi-Lingual) • Have the wherewithal to Service the Minimum of 1 Practice Site. Days and hours will vary by location (Saturday & Evening Flexibility); Must be open 24 hours and 365 days per year. • Consulting laboratories shall submit the following sample reports with their proposals: • Analytics Report • Validity Testing including testing for masking agents and counterfeit urines • Consistent/inconsistent Medication Reports • Sample report with embedded PDMP data o Results to be available within 24-48 hours of collection • Consulting Laboratories shall also meet the following IT Requirements: • Bi-Directional Interface • Pay for Interface • Supply implementation team • Provide Escalation List • Furnish office supplies, printers, computers for phlebotomist/collectors • HL7 Compliant • Compatibility with EHR • Ability to connect remotely to our server • Be familiar with our EHR • 24/7 IT Support • EDI, VPN Network built into our System • Software (may have an additional cost) that can provide population analytics to assist in Population Health Management and Quality Reports. Please elaborate on specific indicators your software can track and specify if it can be modified to be site/provider specific. Also, provide information on how your software can integrate with our Electronic Health Record software. • Consulting laboratories shall also l identify other entities that they have serviced, and they shall include a disaster preparedness plan of action.
