Specifications include, but are not limited to: Services to be funded through the Area Plan Contract include, but may not be limited to: Information and Assistance, Outreach, Transportation, Assisted Transportation, Telephone Reassurance, Residential Maintenance, Housekeeping/Homemaker, Homesharing, Care Management, Certified Home Health Aides, Visiting Nurse, Hospice, Medical and Social Adult Day Care, Adult Protective Services, Legal Assistance, Oral Health, Evidence-Based Physical Activity/Education, Physical Health, Friendly Visiting, Language Translation & Interpretation, Mental Health, Socialization/Recreation, Assistive Technology, Home Sharing/Matching, Emergency Utility Support and Congregate and Home Delivered Meals; which must include: Congregate: menu planning, food preparation, packaging, delivery and nutrition education, Home Delivered Meals: menu planning, food preparation, packaging, daily delivery and nutrition counseling.
