1.1. The Contractor must provide housing navigation services to individuals in qualified Money Follows the Person (MFP) inpatient facilities determined to be eligible for participation in the MFP program and in need assistance with finding and securing a community-based residence, who have been referred to the Contractor from an approved case management agency and/or the Department (hereinafter “participants”). 1.2. The Contractor must ensure services are available in the Northern Region consisting of Belknap, Carroll, Coos, Grafton, Merrimack and Strafford counties and/or the Southern Region consisting of Cheshire, Hillsborough, Rockingham and Sullivan counties. 1.3. The Contractor must provide services during business hours, 8:00 AM to 5:00 PM, Monday through Friday. 1.4. The Contractor must conduct and document a person-centered, face-to-face assessment of the MFP participant’s individual housing related strengths and needs utilizing a housing assessment form approved or provided by the Department within fifteen (15) business days of receiving the initial referral or at a later date, at the request of the participant. 1.5. The Contractor must develop and document an individualized housing transition and stability plan, including identifying housing preferences and potential housing options, based on the completed assessment within five (5) business days of the face-to-face meeting with the participant.
