UMC is looking to find the most qualified Renal Dialysis Coordinator to best meet the needs of the hospital. Services will include the development and implementation of providing patient education, processing timely dialysis placement referrals, and managing avoidance of admissions through the Emergency Department and dialysis on the day of discharge. The Renal Dialysis Coordinator is responsible in (i) educating mild chronic renal failure adult patients and end stage renal disease adult patients; (ii) providing supervision and participation in completion of short and long term care plans; (iii) providing admission of new adult patients according to facility procedure; (iv) ensuring the educational needs of patients are met, and educating patients and their family about End State Renal Disease, dialysis therapy, diet and medication; and (v) arranging outpatient dialysis chairs, obtaining labs and clinical information, and discussing with the patient the location of the outpatient dialysis chair based on their demographics.
