Specifications include, but are not limited to: 1. Provide phlebotomy and laboratory services for DHHS LRC Facility residents as referred by the medical staff of DHHS. 2. Provide laboratory services for residents transported to DHHS LRC laboratory by DHHS LRC Whitehall Facility. 3. Provide a Phlebotomist to DHHS LRC’s clinic area to perform daily scheduled lab draws for DHHS LRC Facility residents’ Monday through Friday, 7:00 a.m. to 1:00 p.m. C.T. with the exception of the following six major holidays: New Year’s Day, Memorial Day, July Fourth, Labor Day, Thanksgiving, and Christmas Day. Phlebotomy services are not provided on these holidays. 4. Contractor shall have use of DHHS LRC Facility provided clinic space to perform the services of this Contract. a. There shall be no fees charged to the Contractor by DHHS for the use of the DHHS clinic space provided. b. All Contractor employees will wear identifying uniforms and/or nametags while on duty within the facility. Proper identification should include the company and employee name. c. DHHS will furnish all necessary facility access to the Contractor. Security access/I.D. cards will be provided to the Contractor and must be worn at all times. A service charge may be assessed against the Contractor for the cost of replacing access/I.D. cards lost through neglect by the Contractor or Contractor’s employees. The Contractor must report lost cards to DHHS contact immediately. d. Contractor shall follow all applicable facility procedures before entering or exiting the facility or while in facility. 5. DHHS LRC Whitehall Facility will transport residents to Contractor’s laboratory facilities.
