· Ability to see same day emergency and/or after-hours patients
· Provide a treatment plan for services over $500
· Accept reimbursement rate listed in contract
· Ability to provide services listed in the reimbursement tables
· Provide referrals for specialists when referring out
· Provide timely invoices
· Provide adequate staffing for dental office
· Provide reasonable hours for patients to be seen
Pediatric Dental office services include:
· Provide treatment for up to 10 children with urgent needs
· Ability to see the same day emergency and after-hours patients
· Provide a treatment plan for each child
· Accept reimbursement rates as listed in the contract

· Provide referral for specialists when referring out
