Description of services attached as part of the RFP package. Please note: 1. This is an online submission. You will need your MITN password and 4 DIGIT PIN in order to submit your proposal. If you do not have the one or either you will need to contact the MITN Help Desk for assistance. Some of the RFP documents have been created as form fill for your convenience. You will need to complete the cover sheet, attachments C & D and provide examples of your photographic work online. You should complete the cover sheet as well as attachment C&D save them to your computer desk top or hard drive. When you are ready to upload the documents and examples of your work you will upload the files into the MITN system prior to the due time and date. If you would prefer to mail examples of your work you may do so by using the mailing label provided. The examples must be received no later than August 21, 2014, 4:30 p.m. Uploading is a very easy process very much like attaching a document to an email. Should you need assistance with the upload contact the MITN Help Desk. 2. Please note the insurance requirements for a Professional Services contract. If you are unable to meet these requirements, please provide copies of your current insurance.
