2025-021 - YOUTH INPATIENT DISCHARGE PLANNING

Through this Request for Proposals (RFP), Detroit Wayne Integrated Health Network (DWIHN) hereby invites businesses who meet the qualifications and specifications set forth herein to submit proposals for the purpose of providing inpatient discharge planning, face-to-face with children who were admitted to an inpatient level of care without an assigned Clinically Responsible Service Provider (CRSP).  Respondents to this proposal would identify children who are currently in an inpatient facility receiving services, visit the children on the inpatient units, and develop a plan of care to transition the children and their families to a CRSP of choice in the community.  Respondents would have mobile capability to respond and coordinate face-to-face and will be able to bill CPT code T1017 when visiting the children on inpatient units.  Respondents would coordinate with inpatient treatment teams, leadership, and DWIHN in order to create a transition plan and supply subsequent data to show resolution over the course of 120 days with face-to-face check-ins every 30 days until case closure.
