Summary of Specifications:
The City of Rochester Hills seeks a company to provide on-site, off-site and/or electronic services to its employees to include: · On-site (at multiple locations) and/or off-site health screenings for weight and body fat measurement, blood pressure, blood work, flu shots, PSA testing, complete blood count (CBC) and personal coaching. · Administer and perform Health Risk Assessments (HRA) · Intervention with at-risk individuals identified by the HRA · Provide measurements tools and results, including access to aggregates and other review identified as appropriate. The company must comply with all guidelines and regulations set forth in the Health Insurance Portability and Accountability Act (HIPAA) and will be required to sign a Business Associates Agreement (BAA). Nothing contained in the BAA is designed to keep confidential any information deemed to be public information by applicable state or federal laws and regulations. The health risk assessment will be provided electronically and completed by employees on-site; however should an employee require paper format, accommodations will be provided by the awarded company. The City of Rochester Hills does not guarantee the actual number of participants. Should a proposer have a minimum number of participants required for any particular service, the proposal shall clearly reflect that it shall be assumed that no minimum participation is required. Intervention shall include, but not limited to, health coaching for diabetic prevention and control, management of cholesterol levels, asthma control, manage and prevent high blood pressure, smoking cessation, weight management and stress management.
