Specifications include, but are not limited to: 1.5.1 DMH intends to implement a web-based, commercial off-the-shelf Case Management solution for HCBS Waiver programs to meet the business needs of the MME. The Case Management solution will support the lifecycle of consumer care including: information and referral, intake, assessment and reassessment, care planning and service authorization, service delivery and payment; case closure, and reporting. 1.5.2 The objectives of the Missouri Enterprise Case Management system are to: a. Improve efficiency and effectiveness of HCBS Waiver programs that will lead to increased productivity and better customer service; 1) Outcome - Decrease processing time for ISP development and authorization by 10%. b. Improve health outcomes for individuals with DD through appropriate supports and services to enhance quality of life; 1) Outcome - Decrease the percentage of Individual Support Plans (ISP) finalized after the implementation date by 10%. c. Improve capability to prevent and reduce fraud, waste, and abuse; 1) Outcome - Improve ability to enforce rate methodology in ISPs and service authorizations based on required assessments. d. Increase staff, client and provider satisfaction by providing on-line capabilities and automated processes; 1) Outcome - Provide online, real-time information for staff, providers, and individuals/families on ISPs and services delivered.
