SERVICES REQUIRED: The Harry S. Truman Memorial Veteran’s Hospital (HSTMVH), Columbia, MO, requires the following services to be provided in a private hospital, office or clinic environment to Veterans, primarily residing in Kirksville, MO and surrounding counties catchment area (including Adair, Knox, Lewis, Linn, Macon, Putnam, Schuyler, Scotland, Shelby, and Sullivan counties).
[X] Primary Care CBOC: offers both medical and mental health care (physically on site and by telehealth) and may offer support services such as pharmacy, laboratory, and x-ray. Primary Care CBOCs are required to provide both primary care and mental health services. Mental Health services will be provided physically on site by contractor staff and by VA staff via telehealth.
PLACE OF PERFORMANCE: This facility shall be located within the city limits of Kirksville, MO in Adair County. Kirksville, MO is 92 miles from Columbia, MO.
