Scope of Services and Project Objectives The Contractor shall propose a plan and forecast the number of individuals and entities to be reached to provide the following services: High Impact Community-Based Testing (HICBT) Community in Partnership The contractor will establish new community testing sites in rural areas of St. Louis, including Lincoln, Warren, St. Charles, Franklin, and Jefferson counties, by collaborating with entities serving any of the priority populations: ● Men who have sex with men (MSM) ● Black women ● Transgender and gender-expansive individuals ● People who use substances ● Partners of people living with HIV ● Individuals experiencing homelessness These partner sites must not be engaged in community testing with other organizations, must not be located within three miles of a current testing site, and must be willing to provide indoor or mobile unit hosting space. Syndemic and Integrated Screening, Testing, and Vaccination Promotion The contractor will provide integrated screening for HIV, STIs (Chlamydia, Gonorrhea, Syphilis), Hepatitis, Tuberculosis, and MPox and will promote vaccination during testing events. The contractor will refer individuals to appropriate health services. As needed, the Contractor will provide PEP services and referrals for those exposed to HIV within 72 hours. HIV Risk Reduction Strategies The contractor will include HIV risk reduction strategies during test result counseling, including, but not limited to, PrEP promotion and referrals. The contractor will maintain a 90% referral rate for HIV-negative clients. All PrEP and PEP referrals should include two points of follow-up contact within 60 days of the referral. Linkage to HIV Medical Care Contractor will: ● Link individuals who test positive to HIV care, treatment, and support services within 10 days. ● Notify the Department of Health and the Missouri Department of Health and Senior Services (DHSS) Disease Investigation Specialists (DIS) within one business day of any positive HIV result. ● Educate all newly diagnosed persons about options to obtain care, including telehealth programs. ● Complete a needs assessment for newly diagnosed individuals. ● Make space available for any persons living with HIV to attend telehealth appointments as needed. ● Follow up with HIV+ clients who have given permission approximately one week after testing to verify if the client has engaged with case management or HIV medical care. Condom Distribution The contractor will recruit organizations and businesses in rural St. Louis counties, including Lincoln, Warren, St. Charles, Franklin, and Jefferson counties, to become condom distribution sites by connecting interested entities with the DOH condom distribution program team. Data Collection and Reporting Contractor will: ● Report all communicable diseases within the required timeframe using the CD-1 form. ● Submit HIV testing data by the 15th of each month. ● Use DOH-approved instruments for reporting and evaluation ● Participate in monthly meetings with the DOH ● Participate in site visits with the DOH
