The contractor shall provide HIV Lost to Care, Retention in Care, and Peer Navigation Services for Missouri residents living with the human immunodeficiency virus (HIV) or acquired immunodeficiency syndrome (AIDS) within the Kansas City, Southwest, and/or St. Louis Regions of Missouri as stated on the Notice of Award issued by the Division of Purchasing for the Department of Health and Senior Services (herein after referred to as “state agency”) in accordance with the provisions and requirements stated herein and to the sole satisfaction of the state agency. The contractor shall understand and agree that the Kansas City Region shall be defined as the counties of Cass, Clay, Clinton, Jackson, Lafayette, Platte, and Ray. The contractor shall understand and agree that the Southwest Region shall be defined as the counties of Barry, Barton, Cedar, Christian, Dade, Dallas, Dent, Douglas, Greene, Hickory, Howell, Jasper, Laclede, Lawrence, McDonald, Newton, Oregon, Ozark, Phelps, Polk, Pulaski, Shannon, St. Clair, Stone, Taney, Texas, Vernon, Webster, and Wright. The contractor shall understand and agree that the St. Louis Region shall be defined as the counties of Franklin, Jefferson, Lincoln, St. Charles, St. Louis, St. Louis City, and Warren. The contractor shall collaborate and coordinate with other Missouri Ryan White grant recipients, all HIV case management providers, and all HIV case management supervisors and trainers within the contractor’s respective awarded region to coordinate all parties’ efforts and resources to avoid duplication of services, and to assure the provision of efficient, cost effective, quality services.
