Specifications include, but are not limited to: Complete 42 projects in Hennepin and Ramsey Counties during the contract period. The CONTRACTOR will perform the following deliverables: A. Marketing a. Market the program through community connections and resources b. CONTRACTOR and HCHRA will cooperate on maintaining any program referrals, inquiries and enrollment through the program’s SharePoint B. Eligibility determination a. Collect applicable grant documents and income verification from eligible households b. Complete an individualized assessment of each home C. Design a. Propose a wood design appropriate for the site and disability of each recipient. Design should be in accordance to “How to Build Ramps for Home Accessibility”. i. Ramps are built with a slope no greater than 1:12 ii. All steps are built with a tread length of 34” and riser heights between 3.5” and 5” iii. Can be constructed throughout the year iv. Ramp/step design should be modular, portable and reusable b. Provide cost estimate of proposed design c. Provide a to-scale existing site drawing and proposed ramp/step design drawing D. Case Management a. Submit eligible households proposal packet to HCHRA through SharePoint, to include: i. Case Narrative ii. Grant Application iii. Grant Agreement iv. Tennessen Warnings v. Income documentation vi. Ramp design proposal and cost estimate b. Submit necessary closing paperwork including before and after pictures i. Signed work contract (Exhibit A) ii. Signed sworn construction statement iii. Signed completion certificate iv. Invoice v. Before and after pictures E. Construction a. Manage project schedule i. Construction work will not begin until Scope is approved by HCHRA ii. Onsite construction will not exceed 10 business days iii. Projects to be completed within 3 months of approval b. Obtain and store necessary materials
