Specifications include, but are not limited to: Complete psychiatric or psychological assessments of residents upon the request of Home. Such assessments shall include documentation of past history, current status, and information regarding expected outcomes and residents' potential to achieve the highest level of function. Communicate the results of assessments to designated staff of Home and provide appropriate documentation of the assessments for residents' medical records at Home. Special testing requiring additional resources must be evaluated and approved by Home’s Medical Director or designee on an individual basis. • Prescribe and order appropriate medications, treatments, instructions, counseling and modalities that address residents’ psychiatric and behavioral status and needs, and promote the highest level of resident function. Communicate any such orders and prescriptions to appropriate staff at Home, and shall provide adequate documentation of all such prescriptions and orders for residents' records at Home. • Provide on-site services as requested and be available for telephone consultations as resident needs dictate, respond in a timely fashion to notification by Home of resident problems or changes in condition or status, and order appropriate monitoring, tests, treatments or transfers. • Communicate to residents and resident representatives, as requested and appropriate, information regarding condition, prognosis, treatment options and likely outcomes of treatment. Any assessments or other documentation that would be detrimental to the resident if the resident were to view them should be clearly marked as confidential; if maintaining data as confidential, clearly mark such information, and provide written reasons why the data cannot or should not be shared with the resident who is the subject of the data. • Assess current and future mental health service needs with an emphasis on resident centered or directed care.
