Specifications include, but are not limited to: • Individuals live as independently and autonomously as possible. • Program is based on the “Housing First” model of supportive housing. • Individuals have their own independent full apartment or other independent housing. • For waiver service participants under a master-lease, it is required that a plan is developed that addresses the transition of the master-lease into the participants name within 2 years.
