Specifications include, but are not limited to: Dakota County Community Services is seeking to contract with one or more community agencies to provide case management for individuals with disabilities and older adults who receive funding through one of the following home and community-based services programs and/or county funded case management: Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC), Brain Injury (BI), Developmental Disabilities (DD), Non-Waiver County Funded Rule 185, Elderly Waiver (EW) or the Alternative Care (AC) program. This RFP is intended to grow capacity of contracted partners to support cases across all of the above programs and also to recruit more specialty providers, including the following: Culturally specific or special niche population/responsive agencies, able to effectively serve various cultural communities, including but not limited to East African, Hmong, Latino, LGBTQ, Older Adults, and Indigenous communities, as well as people who are deaf/hard of hearing Agencies with expertise in serving CAC cases (people with significant medical needs and hospital level of care) - these cases have not previously been contracted in Dakota County Case management services include but are not limited to, informing the individuals or the individual’s legal representative of service options, identification of potential service providers, development of a service plan based on the individual’s assessed needs and preferences, accessing and coordinating services, and evaluation and monitoring of services identified in the individual’s plan. Case management for these individuals will require travel throughout the Metro area
