1. Title I—Academic Instructional Services a. Provide a description of your proposed model for providing high quality instructional and/or related support services to Title I eligible nonpublic students. b. Provide a description of how you evaluate the effectiveness of the services provided. The description must include evidence or data demonstrating your organization’s ability to effectively meet the needs of Title I eligible nonpublic students, how teachers/staff make data-based decisions regarding student progress, types of information used to make decisions about services provided, and how decisions are made using the information described. d. Provide a description for how your organization communicates with district officials, families, and nonpublic school staff regarding student progress data of Title I eligible nonpublic students. 2. Title I—Family Engagement a. Provide a description of the proposed model for family engagement of parents/guardians of Title I eligible nonpublic students. b. Provide a description for how your organization would plan to provide one or more of the following activities: i. communication with parents of each participating student about academic progress ii. school-parent compacts iii. newsletters iv. parent and family engagement meeting(s) v. parent and family training activities and materials vi. home learning materials and activities vii. other parent and family engagement activity (specify) c.
