The selected provider would be expected to provide community residential services for one adult male receiving support services through the Developmental Disability Waiver. The provider would also be required to provide additional support services unique to the individual including, but not limited to, behavior support, medication administration, feeding and hands on assistance to complete most activities of daily living. Service Expectations 1. Maintain a 2:1 staff ratio. 2. Provide overnight staffing ( 2 awake staff). 3. Engage with resident and help them make the home their own by discussing décor, schedules, community events. 4. Encourage and facilitate community outings if wanted by resident. 5. Creative, innovative, and person-centered techniques and approaches must be utilized to include careful planning and attention to the environment. 6. Recreation and leisure coordination will be needed, including the facilitation of natural supports, and mentoring to develop social skills, improve self-esteem, and maximize community membership. In-house programing will also be needed to provide a daily structure. 7. Supports and facilitates appropriate connections with family members. 8. Home modifications to support property destruction and have a safe environment 9. The home should have the following features: located away from busy roads or highways; have a fenced in backyard; furniture secured to wall/floor; reinforced walls in bedrooms and main living space; and alarms on doors.
