The Minnesota Department of Veterans Affairs (“MDVA” or “State”) is seeking proposals from qualified organizations to provide prosthetist and orthotist (“P&O”) services, including the recommended durable medical equipment, that meet the clinical needs of veterans and their spouses who are residents of the Minnesota Veterans Homes(“MVH” or “Homes”). The selected vendor must meet the minimum requirements for background and experience and be able to provide comprehensive delivery of P&O services, to include the recommended durable medical equipment either onsite at the Home or via a telehealth program utilizing secure technology, depending upon the individual clinical needs of each resident, and in coordination with the Rehabilitation Coordinator or Director of Nursing of each facility. Durable Medical Equipment (DME) refers to medical equipment that is medically necessary and prescribed by a healthcare provider which is designed to be used long-term or for repeated use. For this purpose, MDVA is seeking an experienced vendor and proposals that meet the specialized and custom service delivery and equipment needs for residents such as specialized/custom wheelchairs, ankle-foot orthosis (AFO), artificial limbs, and diabetic shoes as common examples, but not an all-encompassing list. The MDVA operates continuously and is responsible for the health and well-being of more than 800 residents across its eight Minnesota Veterans Homes. Each Home is licensed by the Minnesota Department of Health (“MDH”), and all are included within the scope of this solicitation as listed below: • MVH – Minneapolis o Skilled Nursing Facility – 300 Licensed Beds o Domiciliary Program (Boarding Care Home Facility) – 50 Licensed Beds o Adult Day Center – 35 Licensed Beds • MVH – Hastings o Domiciliary Program (Boarding Care Home Facility) – 144 Licensed Beds • MVH – Luverne o Skilled Nursing Facility – 64 Licensed Beds • MVH – Fergus Falls o Skilled Nursing Facility – 85 Licensed Beds • MVH – Silver Bay o Skilled Nursing Facility – 54 Licensed Beds • MVH – Montevideo o Skilled Nursing Facility – 72 Licensed Beds • MVH – Bemidji o Skilled Nursing Facility – 72 Licensed Beds • MVH – Preston o Skilled Nursing Facility – 54 Licensed Beds The Minnesota Veterans Homes currently offers a full range of rehabilitation services under skilled nursing facility and boarding care home licensure and seeks to partner with an organization or organizations that are capable of meeting the clinical needs of its resident population when referred for P&O services. Responders must demonstrate compliance with all applicable state and federal regulatory and credentialing requirements necessary to fulfill clinical referrals. The MDVA prefers to award a single contract to a vendor capable of providing the full scope of P&O services across all eight Homes, delivered either onsite or via telehealth in accordance with resident-specific clinical needs, however, will consider proposals for servicing of one or several homes in a specific geographic region.
