The Department plans to award HVAC Program grants to eligible entities operating within the state of Minnesota. Award funds will be utilized to reimburse expenses incurred for the following activities, (collectively called “HVAC Program Work”): 1. Assessment Report: A heating, ventilation and air conditioning assessment report prepared by a skilled and trained workforce and/or a qualified contractor. 2. Engineering Report: A report verified by a licensed professional engineer that includes: a. Costs of adjustments or repairs necessary to meet ventilation and filtration requirements; b. Determination and recommendation of whether any cost-effective energy or electrification upgrades or replacements are warranted; and c. Recommendations for additional maintenance, replacements or upgrades to enhance safety, performance or reduce greenhouse gas emissions. 3. HVAC Verification Report: A report prepared and certified by school facilities staff or a contractor that verifies compliance with local and state guidance and includes the following information and documentation: a. Name and address of the school facility and individual preparing and certifying the HVAC Verification Report; b. Verification of the contractor’s name; c. Verification of initial operating condition; d. Verification that all construction work has been performed by a skilled and trained workforce; e. Description of the outcome assessment, maintenance, adjustment, repair, upgrade and replacement outcomes resulting from work in Activities 4, 5, 6 and 7 below; f. Documentation of Activity 5.a. and the MERV rating; g. Verification of Activity 5.b; h. Verification of Activity 5.c; i. HVAC system deficiencies;
