Specifications include, but are not limited to: 1. Medical Surveillance Services A program managed in accordance with 29 CFR 1910.120/.134/.1001 and numerous substance specific U.S. Occupational Safety & Health Administration (OSHA) standards and consisting of medical evaluations, medical screening, reporting, recordkeeping, and Physician or other licensed health care professional recommendations with the intention of identifying, controlling, and mitigating risks associated with employee exposures to physical, chemical, and other workplace hazards. The Contractor will perform Medical Surveillance Services in accordance with 29 CFR 1910.120, 29 CFR 1910.134, and 29 CFR 1910.1001, and other standards, as identified by the MPCA, and promulgated by OSHA, or the Minnesota OSHA Division of the Minnesota Department of Labor and Industry.; a. Baseline and Exit Examinations; This task shall include: • Obtaining, reviewing, and evaluating medical and occupational histories of each participant in the MPCA Medical Surveillance Program. Employee Medical Evaluation Questionnaire shall be collected on a form which has been reviewed and approved by the MPCA’s Authorized Representative; • Baseline and exit examinations, to be performed by or under the supervision of a licensed physician, and the scope of which will be in-accordance with applicable regulatory standards identified in 2. Sample Tasks and Deliverables, 1. Medical Surveillance Services above; • Follow-up examinations, at the discretion of the examining physician, when an employee is exhibiting known or suspected adverse medical conditions potentially associated with occupational exposures. The contractor shall notify the MPCA’s Authorized Representative anytime follow-up examinations are required.; b. Periodic Examinations; This task shall include: • Obtaining, reviewing, and evaluating medical and occupational histories, and occupational exposure logs of each participant in the MPCA Medical Surveillance Program. All histories shall be collected on a form which has been reviewed and approved by the MPCA’s Authorized Representative; • Periodic examinations, conducted at a frequency applicable to the respective employee, to be performed by or under the supervision of a licensed physician, and the scope of which will be in-accordance with applicable regulatory standards identified in 2. Sample Tasks and Deliverables, 1. Medical Surveillance Services above; 2. Quantitative Respirator Fit-Testing An assessment of the adequacy of respirator fit by numerically measuring the amount of leakage into the respirator. QNFT will be provided annually to each employee who has been issued a tight-fitting respirator by the MPCA. All fit-testing will be conducted in accordance with 29 CFR 1910.134, the contractor will be responsible for providing any adapters which may be required for accomplishing QNFT. Tight-fitting respirators currently used by MPCA include: • 3M 5000 Series; • 3M 6000 Series (full-face and half-face); • 3M 7800 Series full-face powered air purify respirator (PAPR); • 3M 8576 filtering facepiece
