The County of Hennepin, State of Minnesota (“County”) is soliciting proposals for Conditional release services for the Department of Community Corrections and Rehabilitation (DOCCR) Adult PreAdjudication Services (APAS) Division. Services are provided to adults who are ages 18 and older who have a pending adult criminal charge(s) in Hennepin County, who have been court ordered for conditional release supervision, and are considered high risk to reoffend per the bail evaluation. This RFP seeks proposals that reflect the conditional release supervision model, delivering culturally specific, trauma-focused, and person-centered supports that include but are not limited to housing navigation, assistance accessing employment, obtaining health insurance and accessing health care, addressing food insecurity, navigating transportation, and accessing childcare. Services must be accessible in person and remotely, depending on the nature of the service(s) provided. Additionally, the service(s) must comply to the client’s court-ordered conditions.
