The selected provider would be expected to be a currently licensed 245D provider who is able to be licensed as a child foster care provider. The individuals identified for this home have specific and significant needs that the selected provider would need to demonstrate their ability to address safely and effectively. The provider would also be required to provide additional support services unique to the individual including, but not limited to, behavior support, medication administration, and frequent supervision. 2.1. Mandatory Services The successful provider will have the ability to perform all the functions under mandatory services, including: • Provider has a current 245D HCBS license • Provider has familiarity with Child Foster Residence Setting requirements. • Provider is familiar with trauma informed care and has staff who are trauma informed and able to utilize strategies such as de-escalation and emotional regulation. • Staff trained in Reactive Attachment Disorder, Post Traumatic Stress Disorder and Neurodevelopmental Disorders. such as Fetal Alcohol Spectrum Disorder and Attention Deficit/Hyperactivity Disorder • Staff trained in maladaptive behaviors such as high-risk sexual behaviors, suicidal/homicidal behavior and/or aggression toward people and animals. • Provide 24/7 staffing including awake overnight staff. • Engage with resident and help them make the home their own by discussing décor, schedules, community events, meals, etc. • Work with and follow any recommendations made by therapists and other mental health professionals. • Encourage and facilitate community outings if wanted by resident. • Ensure community appointments are attended. • Ensure attendance at school and required school events. • Creative, innovative, and person-centered techniques and approaches must be utilized to include careful planning and attention to the environment. • Recreation and leisure coordination will be needed, including the facilitation of natural supports, and mentoring to develop social skills, improve self-esteem, and maximize community membership. In-house programing will also be needed to provide a daily structure. • Supports and facilitates appropriate connections with family members.
