Specifications include, but are not limited to: Individuals with Disabilities Act (IDA) Intake/Evaluator services for the Health Department’s Toy Talker Program. The Early On IDA Evaluator shall furnish the program with professional services as the Early On IDA Evaluator of the Jackson County Health Department and as outlined in the goals and objectives of the Early On ICC. -Receive referrals to the PAT Program -Contact referred parents within 10 business days to schedule evaluation and intake. -Conduct an IDA evaluation, and determine entry Child Outcomes Survey Form (COSF) scores for each participating family. -Coordinate services with the PAT Parent Educator/Service Coordinator -Complete all required reports in a timely and comprehensive manner. -Adhere to required confidentiality practices.
