All correspondence must be made through the Vendor Portal. Specifications include, but are not limited to: 1. The Utica Community Schools comprehensive accident policy will be a non-roster school hour program and applicable to all students enrolled in Utica Community Schools programs. A. This includes all K-12 students as well as children enrolled in Utica Community Schools sponsored preschool and child care programs. B. The definition of covered programs shall include those students enrolled in co-op and work study programs while on employers site. C. Direct travel to and from school, up to one hour, also included. D. Coverage shall include all athletic programs including but not limited to high school football. 2. The initial premium will be paid based on estimated enrollment of 26,084 students, subject to adjustment after the official enrollment count is established in September. 3. All proposals must be completed, utilizing the attached form, indicating dollar cost per student and summary of coverage. 4. This is an excess plan designed to respond over and above other insurance available to the student and his/her family. It should pay on an excess basis medical expenses from the first dollar to the maximum stated in the policy. A. Provide pricing options at different deductibles and medical maximums listing on the excel vendor pricing sheet. 5. The successful insurance carrier must administer all claims. Claims administration will be coordinated with the Director of Purchasing and Risk Management.
