SCOPE LTBB, through grant funding from Indian Health Services, provides health care to Tribal Citizens within our service area. The LTBB Health Department (Health Park Building) is located at 1260 Ajijaak Ave., Petoskey, MI 49770. In order not to disrupt patient care this work will need to be completed after hours and equipment may need to be kept off site if impedes client parking or traffic flow. A. Supply all equipment and materials necessary to complete project objectives. B. Work with LTBB Health Department to schedule work being performed to work around event schedules. C. Erect safety barriers and signage to cordon off work area for safety, when needed (i.e. for weekend work). D. All work shall be in accordance with state, local and tribal building codes and standards. E. All work is performed in accordance with the applicable codes, safety standards, and adhere to the general provisions of the contract documents F. All work Must be performed after hours being nights and/or weekends. G. Asphalt repair - clean and fill holes, prime oil spots, edge and seal both front and rear lots. H. Fill and seal all cracks in parking lot. I. Seal entire area of both front and back parking lots only. J. Stripe both parking lots. K. Project is subject to Davis Bacon and Related Acts (40 USC §276a; 29 CFR Parts 1, 3, 5, 6 and 7), and Certified Payroll Records must be submitted to LTBB during the project period as noted in item B of Section II.
