The LTBB Health Clinic is located in the Health Park Building at 1260 Ajijaak Ave., Petoskey, MI 49770. LTBB desires commitment from a Psychiatrist for all five (5) grant years with year 1 being compacted and with the understanding that each year is contingent upon yearly awarded funding and need. A. Must be Licensed and Certified by State of Michigan. B. Provide virtual, in person, or combination of the two services to LTBB Health clients up to 24 hour per week. C. Onsite service will be at the LTBB Health Department located at 1260 Ajijaak Ave, Petoskey Michigan, D. LTBB shall assign clients as needed. E. Services may consist of: o Clinical Psychiatry Services o Care coordination, o Collaborative Care Model with Primary Care o Behavioral Health, Pharmacy o Traditional Healing o Cultural Advisor F. Collaborative Care Model of care where recommendations for Psychiatric care is made to Primary Care who is managing the patients care. G. Provide Holistic, Integrative Care, that is culturally adapted, to meet patients and families where they are, for best shared decision making. H. Prepare and submit reports as requested. I. Comply with the national standards for the privacy of individually identifiable Protected Health Information adopted by the Department of Health and Human Services ("DHHS") pursuant to the Health Insurance Portability and Accountability Act of 1996, as published in a final rule dated December 28, 2000, and final rule modifications published January 25, 2013, all as amended by the Health Information Technology for Economic and Clinical Health Act (as may be now or hereafter amended or modified, "HIPAA"). J. Contractor agrees to hold harmless and indemnify the Program for any and all acts or omissions of the Contractor, which result in liability to LTBB.
