Specifications include, but are not limited to: Oversee completion of Michigan DNR “Health Risk Evaluation” form and “Health Risk Fitness Evaluation” form by each test individual; forms must be completed onsite. Collect vitals for each test individual: blood pressure, pulse, height, and weight. Conduct resting EKG for each test individual. EKG may be waived at the discretion of the evaluating physician for test individuals retesting within six months of previous EKG. Review “Exercise Warning Signs” form with test individual; obtain test individual’s signature on proper form. Evaluate all information gathered during the pre-test phase and provide Physician’s medical clearance for test individual to participate in fitness test. Contractor shall administer and supervise test exercises as specified in Attachment A - Physical Fitness Testing Guidelines. Lateral pull down: 20 repetitions of 40 lbs. Sit-ups: 11 sit-ups in 30 seconds (minimum standard) Push-ups: 10 push-ups in 60 seconds (minimum standard) Treadmill test: completion of 4 levels, each level at increased incline.
