Tab 1 – Forms: The Proposer must fully complete and include the following forms (See Appendix A2, Required Forms and Affidavits of this RFP), or the proposal will be deemed nonresponsive and rejected without any further evaluation. • A signature binding the offer (i.e. Proposal Form or Cover Letter) and commitment to execute an Agreement with South Lake Schools within fourteen (14) business days after selection; and • Price Sheet, fully completed; and Proposed Fees for the required services: • Proposers are encouraged to propose innovative compensation methodologies, however, each proposal must contain an annual budget for the services for the proposed contract term plus any renewal options. • Annual budgets must, at a minimum, be broken down by type of service to be provided and indicate the estimated hours for those services. • The Price Sheet is to be used to document proposed wage rates, costs and markups during the proposed term • The proposal must contain hourly rates for persons whom the Proposer plans to compensate on that basis. • NOTE: Alternate pricing formats of the Proposers choosing may be provided, however, the Price Sheet of this RFP MUST be completed in its entirety and submitted with the Proposer’s proposal or the Proposer will be deemed non-responsive and Proposer will not receive any further consideration for award. • Verification of Minimum Qualifications Form (or provide the required information to show the Proposer meets each of the Minimum Qualifications as listed in this solicitation document); and • Business Information Questionnaire • Proposer must certify that it neither (a) engages in investment activities in the energy sector of Iran, including the provision of oil or liquefied natural gas tankers or products used to construct or maintain pipelines used to transport oil or liquefied natural gas for the energy sector of Iran, nor (b) extends credit to another person or entity, if that person or entity will use the credit to engage in investment activities in the energy sector of Iran; and Subcontractor Form Note: If the Proposer anticipates utilizing a subcontractor(s), then the Proposal must include information about each subcontractor that will be utilized in the contract. Subcontractors cannot be added or replaced after submittal of the proposal without the prior written approval of South Lake Schools. Proposers are financially responsible for subcontractors that provide support services in proposals. The District will only be responsible for paying the primary Proposer. Tab 2 – Experience and Qualifications: Submit information that substantiates the Proposer meets each and allthe minimum qualifications of this RFP. Complete the Verification of Minimum Qualifications Form or provide the required information for each minimum qualification listed. A reference contact or document that can verify that the Proposer has met the minimum qualifications is also required for each minimum qualification listed in this RFP. • Describe the experience the Proposer has providing similar operations and custodial services, and include the number of years providing similar services. • Describe the experience and qualifications of key personnel and the Site Manager that will be assigned to this project. Include detailed resumes for each key personnel and Site Manager. Include education, professional certifications and relevant past experience. Experience will not be considered unless complete reference information are provided. • State your total dollar volume of business for each year for the past three (3) years. Additionally, please include either a bank reference or letter of credit. • State whether any contracts to which the Proposer was a party has ever been terminated early. If so, identify which ones and provide details. • State whether Proposer is a party in a legal claim or lawsuit with any client as a result of Proposer’s operation of a public service. If so, describe the issues. • Provide a brief description of all litigation for the past five (5) years, including the prime proposer and any team member firms that were: (a) a debtor in bankruptcy, or (b) a defendant in a lawsuit for deficient performance under a contract and the damages claimed exceeded one million dollars ($1,000,000), or (c) a respondent in an administrative action for deficient performance on a project, or (d) a defendant in any criminal action. • State whether Proposer, or any entity in which Proposer has had an ownership interest, ever had a bond or surety canceled or forfeited. If yes, state the name of bonding company, date and amount of bond and reason for such cancellation or forfeiture. • State whether Proposer, or any entity in which Proposer has had an ownership interest, ever been declared bankrupt. If yes, state date, court jurisdiction, amount of liabilities and assets, type (i.e. Chapters 7, 11, 12, etc.), and resolution or current status.
