Service #1 of 1: Wraparound Coordination Services a. Activities the Contractor must perform: The Contractor must: Provide a supervisor assigned to: 1) Oversee the coordination of the Wraparound process. 2) Maintain certification in Wraparound, supervisors and facilitators must attend the four-day Wraparound Orientation/New Facilitator Training and two Wraparound Trainings annually which vary in topic each year. 3) Meet weekly with the Coordinator/Facilitator. 4) Review and approve by signature and date all required documentation including: Crisis/Safety Plan Vision and Mission Statements Strengths Narrative Needs Assessment Child and Family Team Plan Meeting Minutes Transition Plan Graduation Summary 5) Require staff maintain flexible schedules to accommodate the family’s work and school schedule and be available for crisis intervention to families 7 days a week/24 hours per day if necessary. b. Provide a Coordinator/Facilitator to facilitate the Wraparound process. The Coordinator/Facilitator will attend the 4-day New Facilitator Training for certification and attend two additional Wraparound trainings per year to maintain certification. Training attendance shall be maintained in a training log. c. Accept referrals and create Child and Family Teams. All referrals are presented to the Community Team for recordkeeping purposes. In coordination with the family and the case manager from the referring agency, schedule, notify, and arrange location for the Child and Family Team meeting. Coordinate the strength-based discovery of children and their families. This discovery is an assessment of the strengths exhibited by the family. These may be gleaned from both observations and interviews with the family. The Contractor shall use visual planning techniques (i.e. bubble chart, story boarding or other planning formats), to identify the child and family’s strengths and needs in multiple life domains. 4) The Contractor shall prepare the family for Child and Family Team meetings, including discussing the meeting format, expectations, and responsibilities. Identify significant people in the family’s life circle to include on the Child and Family Teams. 5) In coordination with the family and the referring agency, invite other members to the Child and Family Team meetings. 6) The Contractor shall conduct and facilitate meetings of the Child and Family Team for each child, which shall: Create the Child and Family Team; Schedule and conduct regular meetings, incorporating process of strength-based planning and needs discovery; Record proceedings. Facilitate the development of the Wraparound Service/Support Plan, including: Identifying resources to meet client needs as determined by the Child and Family Team. Work as a liaison to other agencies that are available within the community to provide components of the Wraparound Service/Support Plan. Assist the Child and Family Team with development of a creative Wraparound Service/Support Plan. Facilitate discussion of the Child and Family Team to determine how and by whom identified services shall be obtained. These services/supports can be traditional categorical services or services tailored to meet the family’s needs. Complete and distribute the Wraparound Service/Support Plan to the Child and Family Team members, referring agency and Community Team when the family consents to share this information in writing. Develop services to facilitate the Wraparound Service/Support Plan and work with referring agency staff and Community Team in seeking out services/supports tailored to meet the family’s needs as identified by the Child and Family Team. Review the Wraparound Service/Support Plan monthly with the Child and Family Team, formally on a quarterly basis. Any barriers or needs may be presented to the Community Team to assist in attaining outcomes.
