Specifications include, but are not limited to: Vendor will be providing evaluation, clinical assessment and on-site treatment of all wounds with clear and concise documentation weekly for each Veteran seen. Vendor will provide education and training to staff during these weekly rounds and through additional in-servicing. - Round on a weekly or as needed basis with a nurse designated by the Facility. - Perform patient consultations and follow-up care, including but not limited to wound debridement, incision and drainage, skin biopsy. - Develop and execute care plans in partnership with facility staff. - Take part in Quality Assurance programs and other staff programs, including but not limited to nursing staff and medical staff education. - Solely responsible for billing the residents and/or applicable third party payers. - Vendor will discuss with Director of Nursing, Assistant Director of Nursing, or CWOCN the progress and program goals of all Veterans seen. - Vendor will also provide credentialing of all Dr.'s and Staff before entering the facility. Surgical Wound Dressings - Vendor will agree to provide surgical wound dressing dispensed to Veterans when deemed necessary by visiting physician. - Request authorization from payers prior to visit for new veterans requiring wound care. - Document veteran status and wound care needs and include specific dressing orders. - Dispense and facilitate delivery of the appropriate dressings directly to the veteran. - Education to wound care nurse on wound treatments including wound dressings. - Provide reporting on wound care program success. Facility Responsibilities - Provide a dedicated nurse to round with the clinician - Inform the resident’s primary care provider of the clinician’s recommendations within 24 hours. - For each resident requiring services, obtain a consultation order from the resident’s primary care provider. - Provide the clinician rendering services with access privileges to the Facility’s internet service.
