Specifications include, but are not limited to: Child Find Activities 1.1 Result: Eligibility is validated Measure: 100% of enrolled children have documented evidence of vision loss or functional vision loss assessment results Specialty vision services 2.1 Result: Needs of enrolled children and families are addressed in a timely manner Measure: Percentage of referred children who receive initial visit from provider within 30 days of referral from an Early Intervention Program 2.2 Result: Specialized services for children with visual impairments are provided in a continuum of settings, with the goal of inclusion of all children to the extent that is educationally appropriate in natural environments. Measure: A minimum of 75% of services provided occur in natural, non-segregated environments. 2.3 Result: Parents are included in all aspects of intensive treatment programs for children with visual impairment Measure: Program carries out an annual assessment of family/consumer satisfaction employing a survey instrument and process engaging in analysis to inform program planning and implementation. 2.4 Result: Providers collaborate with Early Intervention Programs to meet the needs of enrolled children and families. Measure: 75% of client records affirm Specialty Service Provider participation in IFSP meetings with Early Intervention staff. Program Administration 3.1 Result: Staff is trained and supervised appropriately Measure: Average number of in-service trainings per year; number of staff who attain/retain state and national certifications 3.2 Result: Program bills in an accurate and timely manner Measure: Number of monthly PVs that require adjustments Measure: Number of PVs that require client EIIS number corrections 3.3. Result: Data and reports are submitted in timely manner Measure: Percentage of data requests processed accurately and on time Result: Program corrects noncompliance in a timely manner Measure: Noncompliance is corrected no later than one year from identification
