Specifications include, but are not limited to: The Mission of the Department of Developmental Services (DDS) and the Department of Mental Health (DMH) through this RFR is to create a proof-of-concept model of intensive community wrap-around services to support adults in their home and community who are between ages 18 and 30, eligible for DDS community developmental disability support based on autism spectrum disorder (ASD), and also have a Serious Mental Illness (SMI) or other significant mental health challenges. The goal of this shared service delivery model is to maintain overall individual stability and effective functionality for these young adults in their homes and communities. This proof-of-concept project is identified as the DDS/DMH ASD Intensive Community Wrap-Around Services in this Request for Response (RFR). Initial funding for this proof-of-concept proposal is through the CMS-approved ARPA project, Transitional Services for ages 18-25 who have Autism Spectrum Disorder and Behavioral Health Diagnoses.
