A. Meet with the project team (Town Administrator, Assistant Town Administrator, Council on Aging Director and Human Services Director, Designee of Selectboard and others) on a regular basis (monthly) throughout the contract period as needed. B. With the assistance of the project team, identify existing data (census reports, school reports, studies, etc.) that are relevant to developing a Community-Wide Needs Assessment for those most impacted by an absence of services and programs. C. Working with the project team identify key housing and social service agencies and individuals providing services, the services provided, and their sources of funding. D. Conduct focus groups and/or individual interviews of agencies that provide direct services to residents in the target population– see list of suggested interviews. (Anticipate 2 meetings) E. Conduct focus groups and/or individual interviews of residents. (Anticipate 2 meetings) F. Host public forums as needed to supplement information. (Anticipate 1-2 meetings) G. Complete qualitative and quantitative analysis of collected data and information, including an analysis of existing demographics (race, ethnicity, age, gender, income, etc.), immediate needs and trends, demographic projections and potential change in needs due to shift in demographics. H. Prepare a written report to include those items outlined below in the Project Deliverables and to present that report to the project team and the Selectboard.
