The contractor shall provide routine monthly services within 14 days of being awarded the contract. Include a written Integrated Pest Management Plan (IPM Plan). Reference EPA Publication Number 907K21002. Monthly pest control services and report, maintenance of exterior bait stations, interior walkthrough of entire building for pest services. Include a Termite program with semi-annual inspections and maintenance of subterranean in-ground bait sticks. Areas to be services on a Monthly schedule may include (as determined by inspections, IPM and pest activity): Hallways, offices, storage areas, kitchen/cafeteria, basement mechanical room, In-patient areas, exterior bait stations, and other areas as needed. Frequency Inspection and treatment will be provided Monthly at least 12 times per year. Must also be able to provide one-time services as described in Category 3 for bed bugs, bees, rodents, etc. Pests covered: The following pests are included within the scope of this contract for Category 1 (Integrated Pest Management) and Category 3 (Single Use Service): Rats. Mice. Cockroaches. Incidental/occasional invaders entering from out-of-doors. Pantry pests such as Indian meal moths. Birds, bats, snakes, cats, raccoons, skunks, and all other vertebrates other than rats and mice. Termites and other wood destroying insects. Fleas and ticks. Other arthropod pests are not specifically excluded from the contract. Category 3 Single Use Service Scope- This category is not to be used in place of an Integrated Pest Management (IPM) Plan. For a Bidder to be awarded Category 3 (Single Use Service), they must also bid on and be awarded Category 1 and/or Category 2. This category is to be used on a limited basis when the service falls outside of the routine monthly service and termite plan. Services in Category 3 are limited to: Problem Animal Control to include: Birds, bats, snakes, cats, raccoons, skunks, geese, and all other vertebrates other than rats and mice. Stinging Insects Occasional emergency use for Bed Bug eradication. Service frequency: Contractor must conduct monthly inspection and treatments for hospital and group home. The contractor must be on call for emergency situations, with a response time of 24 hours from request. Service Locations: Include but not limited to Exterior perimeter Entry points Kitchen and break rooms Rest rooms Mechanical and electrical rooms Storage areas and Janitorial areas Office areas Dumpsters and trash enclosures Any areas identified by contractor or Facilities Director Service Methods: Integrated Pest Management Use of non-chemical methods where possible Baiting systems for rodents and insects Sealing of cracks and crevices as part of exclusion work Reporting and Documentation: Service reports after each visit must be submitted to the Facility Director MSDS (If needed) Annual trend reports and recommendations Health and safety compliance logs Any recommendation or added service recommended by contractor must be approved by facility director or maintenance foreman. Contractor Responsibilities: Licensed and insured technicians Provide all tools, materials and equipment Maintain confidentiality and safety standards Train onsite staff on pest prevention upon request
