Specifications include, but are not limited to: a) Provide Title 5 System Inspector and Soil Evaluator training and certification, and recommendations for approval, pursuant to 310 CMR 15.017 and 310 CMR 15.340. b) Provide the following Title 5 Program Services: o Schedule, plan, and/or coordinate Title 5 System Inspector and Soil Evaluator training courses and certification exams in coordination with the Department, pursuant to 310 CMR 15.017 and 310 CMR 15.340. o Administer and/or proctor exams for Title 5 System Inspectors and Soil Evaluators. o Maintain the training, exam, eligibility, and recommendation for approval records of all applicants and electronic databases of certification approval of Title 5 System Inspectors and Soil Evaluators. o Review credentials and eligibility of applicants and register applicants for trainings and exams. o Host website and post Title 5 website course notices, contact hour courses for Title 5 System Inspectors and Soil Evaluators, a list of all approved System Inspectors and Soil Evaluators and other, Title 5 related information. o Provide course materials and coordinate with the Department to update course and exam materials, subject to Department approval, as needed. o Evaluate and present for Department approval a training, exam, and certification fee structure to make these Program Services self-supporting. The program shall be selfsupporting based on the collection of fees associated with training and certification services. o Review applications and submit certification approval recommendations to the Department for initial certification approvals. o Issue the Title 5 System Inspector and Soil Evaluator approval certificates after signature by the Department. o Provide annual reports to the Department regarding the number of courses and exams held, number of TCHs approved, number of participants, number of initial certifications and renewals issued. o Provide an annual revenue and expense report to the Department. c) Document Retention and Annual Reporting. Maintain all records created in the implementation of this Scope of Services for a period of 7 years following document creation, except that records related to a specific individual shall be maintained for a period of 7 years following the expiration or termination of such individual's certification approval. Records required to be maintained hereunder include, without limitation, training and exam results, attendance records, and related databases, and records and databases related to the certification approval process. The original or a true and accurate copy of all such records shall be provided to the Department upon request.
