a. The Bidder shall provide delivery and removal services for oxygen cylinders to Clifton T. Perkins Hospital Center (CTPHC). Delivery shall be on an as-needed basis with a start of a minimum twenty-five (25) oxygen cylinders in place. The delivery amounts are not guaranteed per month or during the contract. b. Deliveries must be made between the hours of 8:00 am – 4:00 pm, Monday through Friday to the Medical Clinic. No deliveries shall be made on weekends or holidays unless arrangements made through the Contract Monitor or designee due to emergency circumstances. Deliveries shall exclude State Holidays as follows: New Year’s Day Birthday of Dr. Martin Luther King Jr. Presidents’ Day Memorial Day Juneteenth Independence Day Labor Day Columbus Day Election Day Veterans’ Day Thanksgiving Day American Heritage Day Christmas Day c. The Contractor shall have written equipment preventive maintenance policies and procedures and will maintain a system for tracking all equipment preventative maintenance by model and serial number. The safety and working conditions of the equipment shall be monitored on an ongoing basis, including maintaining preventative maintenance logs, calibration logs, recalls, logs of oxygen cylinder lot, batch numbers, and the results documented by serial numbers. This documentation will be maintained for the term of the contract.
