(Within the first two weeks) • Develop and send a facility wide announcement to all staff introducing the initiative and outlining opportunities to participate. (Within the first month) 2. Develop and deploy staff surveys by the completion of the fifth month of contract term: • Design and distribute a comprehensive staff survey (paper and digital format) to all SHC employees, with a goal of reaching at least 40% participation (260+ responses). • Survey questions should explore current alignment with existing values, identify challenges, and solicit ideas on SHC’s identity and future priorities. • Provide survey response summaries to SHC leadership. 3. Conduct roundtables, interviews, and Town Halls by completion of the fifth month: • Conduct 10–12 interviews with key stakeholders, including the CEO, CD, COO, DON, and selected department leaders. • Facilitate 6–8 roundtable sessions (8–15 staff each) representing different disciplines and shifts, ensuring broad representation. • Host at least one town hall meeting (in person or virtual) for all staff to provide real time feedback and discuss preliminary themes.
