Specifications include, but are not limited to:The Contractor must operate a comprehensive program of affordable, permanent supportive housing for 21 formerly homeless tenants without children. The property will be located at 8106 Georgia Avenue, Silver Spring, MD 20910 (the site). This program will be known as “Progress Place PLQ”. A minimum of 11 units must be designated for the chronically homeless adults that meet the HUD definition or must meet the Continuum of Care (CoC) approved definition of “vulnerable” defined as: a. Homeless for at least six (6) months (180 days) within a fifteen (15) month timeframe as documented through contact in an emergency shelter and/or outreach program. and b. Have at least one of the following at risk qualifiers: More than 3 hospitalizations over a year 60 years of age or more HIV+/AIDS RFP #1065572 Revised 03/16 14 Kidney Disease/End Stage Renal Disease or Dialysis Liver Disease/ Hepatitis C/ Cirrhosis/ End Stage Liver Disease Chronic and persistent mental health, substance use, or co-occurring disorder. The resulting Contractor must utilize documentation noted in the Homeless Management Information System (HMIS) to verify client eligibility for services. B. The Contractor must accept referrals from the County’s Continuum of Care’s Housing Prioritization Committee, composed of staff members from DHHS, public housing authority, non-profit outreach program partners, and non-profit homeless service providers. The County has the right to make direct referrals into the program if space is available. The County's direct placement may take precedence over those on the housing vulnerability list, maintained by the CoC
