The Contractor shall identify a Care Coordinator to work with the 1:1 Project Manager and the Contract Monitor to communicate any changes or concerns with service delivery. The Care Coordinator shall be available at the request of the 1:1 Project Manager and the Contract Monitor to attend meetings, discuss individual cases, and address concerns. Preferably, the Care Coordinator will not be the Clinical Project Manager. The Care Coordinator shall assist with scheduling, record keeping, and assign children to 1:1 staff based on their clinical needs. Clinical Project Manager - The Contractor shall identify a Clinical Project Manager who shall be a Licensed Certified Social Worker – Clinical or greater and will be responsible for reviewing the child’s behavioral health plans and ensuring the proper treatment plans are in place and evaluating their progress. The Clinical Project Manager also provides clinical supervision to the 1:1 Services staff.
