A. The Contractor shall provide to WMHC a qualified Licensed Nursing Home Administrator as follows: 1. Possess a current Maryland license to practice as a Nursing Home Administrator. A copy of the license shall be maintained in the Administrator’s file and visibly displayed in the Lobby of the WMHC with the current license for the facility. 2. Three (3) years experience as a Nursing Home Administrator in a facility with a minimum of thirty-five (35) beds. Provide three references with contact name, phone number, email address and locations of previous experience. 3. Comply with the Code of Maryland Regulations (COMAR) for Nursing Homes (10.07.02) and the standards of the Centers for Medicare and Medicaid Services (CMS), Maryland Department of Health’s Office of Health Care Quality (OHCQ) and The Joint Commission (Nursing Care Center). 4. Execute a Release of Information Form and identify any prior disciplinary action(s) by a licensing board for use by the WMHC in reviewing the candidate’s qualifications and background. The investigations include, but are not limited to the following: a. Past work history b. Character c. Education d. Military records B. Prior to beginning services under this contract, the Licensed Nursing Home Administrator shall provide the results of a criminal background check. C. Prior to beginning services under this contract, the Licensed Nursing Home Administrator shall pass a medical examination and submit documentation of the examination prior to his/her assignment at WMHC. Fees associated with this requirement are the responsibility of the Contractor. This examination shall be completed by a licensed physician and shall include at a minimum, a past history of communicable disease, and a statement of general health. D. In accordance with the WMHC Infection Prevention and Control Program and as set forth by the Code of Maryland Regulations (COMAR) 10.07.02.21 entitled Maryland Department of Health, Hospitals, Comprehensive Care Facilities and Extended Care Facilities, Infection Control Program, the Licensed Nursing Home Administrator shall provide proof of the following vaccinations prior to beginning services under this contract: 1. Tuberculosis: Provide proof of administration of a two-step tuberculin skin test. The tuberculin skin test must be completed within the last 12 months and must contain results recorded in millimeters of induration, date provided of each reading, signature of administrator, and the lot number and manufacturer of the purified protein derivative (PPD) used to be accepted; or Chest x-ray completed in the last five years (completed for individuals unable to receive a two-step test). The Nursing Home Administrator must be certified free of Tuberculosis before working at the Center and remain Tuberculosis free for the full duration of the contract. If the negative status changes during the term of this contract, it is the Contractor’s responsibility to inform the contract monitor of this change as soon as it is known. 2. Measles, Mumps, Rubella, Varicella and Tdap: Immunity is verified by one of the following: Documented evidence of administration of the vaccine. Laboratory evidence of immunity (except Tdap). 3. Influenza: Annual immunization for influenza, unless refusal due to: Medically contraindicated Religious beliefs In cases of refusal, a declination form must be signed with the reason clearly documented. 4. Hepatitis B Vaccination: OSHA’s Bloodborne Pathogens Standard, 29 CFR 1910.1030, requires that employers offer the hepatitis B vaccination series to any employee who is reasonably anticipated to have exposure to blood or other potentially infectious materials. The hepatitis B vaccination is a series of three injections over a six-month period. 5. *Covid 19: Immunization for Covid 19, unless refusal due to: Medically contraindicated Religious beliefs *not required but strongly recommended and encouraged Western Maryland Hospital Center has the resources to perform prescreening for the antibody immunity for required vaccination for measles, mumps, rubella, varicella, and hepatitis B (except Tdap) at no charge to the individual.
