A. Develop and administer a Board-approved Rehabilitation Program based on its responsibilities, the responsibilities of the individuals it supervises, and the specifications detailed throughout this RFP. B. Develop and abide by a Board-approved process for discharging participants who, after clinical evaluation, do not have a clinical need for treatment. C. Draft and comply with a Board-approved consent form in which the participant authorizes the Rehabilitation Program and the Board to exchange confidential information regarding his/her treatment. D. Develop and comply with a Board-approved consent form in which the participant authorizes the Rehabilitation Program and clinicians to exchange certain confidential information regarding his/her treatment. E. Create and implement a Board-approved Rehabilitation Agreement in which an individual agrees to participate in the Rehabilitation Program by complying with established program policies and an individualized Rehabilitation Plan.
